Admissions Instructions
Pre-Admission:
- Download all necessary admissions forms
- Forms can be filled in, and then printed.
- Sign in all necessary places.

Admission Day
Our Admission Day is Friday unless otherwise arranged with our Admissions Director. Youth and parents are to be at Pinnacle’s main office by 7:00 am. 80 East 1000 North, Suite C, Richfield, Utah. Maps and lodging information are available in our Admissions Packet.
Remember this is an important day for your child, please be prepared in the following ways:
- Compile all necessary items as listed in Admissions Packet.
- Plan ahead on transportation. Some parents bring their child to Pinnacle and other parents arrange for an escort service to bring their child. Our Admissions Director will discuss options with you and give you referrals for our pre-approved teen transportation services.
- Plan on spending the entire day at Pinnacle Foundation with the exception of lunch.
- On Saturday, the parent workshop will start at 8:00 a.m.- 5:00 p.m. Lunch may be catered for a fee of $10.00 per person.
